Who We Are

CASE MANAGER (CM):

We will be directly involved in the mental health of the clients and will provided assistance and coordination of access to community resources. The Case Manager provides follow up to the clients in the are of mental health. The case manager helps individuals, and their families identify the resources needed to access the medical, financial, and educational services that are vital in recovering mental health conditions.

Our Approach

Case Manager Program:

It is the mission of Mental Autonomy Case Management program to assist and work closely with individuals who exhibit  serious impairment due to mental illness and with their families and significant others, and to ensure that they have access to the full array of need and appropriated services resources available in the community.

Additionally, it is the mission of our case manager to ensure that they need of such individuals change, the services and resources shall change in a meaningful manner to address these needs. Though such responsiveness, it is the purpose of Mental Autonomy’s Case Manager to empower their recipients to live a life which is personally meaningful needs for institutionalization, hospitalization, or incarceration, and the enhance their quality of life.

Case Manager serves as means to achieve the welfare and autonomy of the client. It is Highlighted through communications, education, the identification of resources and the facilitation of services. The Case Manager helps identify health providers for better client welfare. The Case Manager service are best offered directly between the case manager, the client, and the staff. Case Manager will assist the client in order to improve the quality of life.
