Program Mission:
It is the mission of Mental Autonomy Case Management program to assist and work closely with individuals who exhibit serious impairment due to mental illness and with their families and significant others, and to ensure that they have access to the full array of need and appropriated services resources available in the community.
Program Vision: It is the Vision of our case manager to ensure that they need of such individuals change, the services and resources shall change in a meaningful manner to address these needs. Though such responsiveness, it is the purpose of Mental Autonomy’s Case Manager to empower their recipients to live a life which is personally meaningful needs for institutionalization, hospitalization, or incarceration, and the enhance their quality of life.
What We Do

Coordination of care with service providers, Monitor medication treatment, assistance to select medical providers and specialist.

Apply for Low-income Housing or Plan 8. Recertification of Plan 8.
03.
Apply for assistant in living or home health services, link with transportation resources and services.
04.
Assistance with legal documentation (residence, citizenship, passport, identification.)
05.
Economic assistance (assistance to obtain food donation, personal and hygiene items donation, households’ toiletries donations, Clothing donation, and furniture donation).
06.
6. Assistance to apply and recertified low income social programs.
Case Manager will assist the client in order to improve the quality of life.
The Case Manager helps identify health providers for better client welfare.
The Case Manager service are best offered directly between the case manager, the client, and the staff.
